Attention To Listing Agents: 

1. Most of the PDF files below are interactive forms. Please download the updated 

    Adobe Reader (free) to view all the files below. 

2. Listing File: Agent must E-mail (or see other option*) your office manager the complete

    listing package in ONE PDF file as soon as your new listing is on the market.

Listing Package Required Documents:

• Listing Report 
• Listing Agreement 
• Consumer Guide To Agency Relationship 
• Agency Disclosure Statement (Signed before an offer) 
• Residential Property Disclosure Form 
• Lead-paint Disclosure (For property built before 1978)
• Sellers Consent To Disclose
• Harmon Homes Ad Form
• Open House Schedule (If applicable)
• Mortgage Pay Off Authorization
• If seller refuse to sign our Consumer Guide above please include Disclosure To Client in your listing report.

Listing Package Optional Documents:

• MLS change form (If applicable. Agent needs to fax this form to the Board at 

                                      419-535-7990 when make status changes on your listings.)

• Home Owner's Warranty (Agent will get $35 by using First American HOW) 

3. Other Option*: Instead of E-mail the Listing File Agent could also physically bring these

    files to the office. Agent needs to pay $199 paper file processing fee for each transaction

    by choosing this option when the listing has been sold.
